It is the responsibility of our Social Care Team to ensure optimal care for our patients following their discharge from hospital. Our Social Care Team works closely with the hospital's physicians and nursing and care staff, as well as working in close cooperation with the relevant authorities, health insurance providers and social service providers. In addition, the Social Care Team also take into consideration the patient's personal situation at home, any family members and the patient's social environment. Prior to the patient's discharge from hospital, the Social Care Team will arrange for rehabilitation, as well as establishing the necessary contacts with self-help groups, charitable organizations and advisory and support services near the patient's home.
