Pomerania e.V., a union of municipalities of the Pomerania region, supports and promotes German-Polish projects that encourage regional development in terms of infrastructure, economy and culture, as well as other areas of social development. One of our areas of collaboration with Pomerania e.V. is the program of home monitoring that we offer to our patients. more

The Fontane project aims to improve the level of care provided to cardiovascular patients in the more rural areas of Brandenburg, which are lagging behind in terms of access to healthcare. The project aims to achieve these improvements by encouraging a cross-sectoral approach that includes the introduction of modern IT systems as well as biomarker-guided diagnosis and treatment. The Brandenburg Heart Center acts in an advisory capacity on all medical matters. more
